First Visit

Medical History - Adult | Medical History - Child | Insurance Information

Thank you for visiting our practice. Please call, or e-mail us to schedule your complimentary orthodontic evaluation. At the time of the evaluation, we will ask you to fill out a medical history form, or you may choose to download and print the form at home and bring it with you to expedite your visit.

New Patient Exam

During the initial exam, Dr. Phoenix will perform a through evaluation of your teeth, bite, oral tissues, temporo-mandibular joints, and jaw relationships. He will discuss your concerns, and will determine your orthodontic needs. We encourage you to ask as many questions as you would like. If treatment is indicated, we will recommend diagnostic records to provide the doctor with more information.

Diagnostic Records

We can usually take diagnostic records on the same day as the initial exam. But, if it is more convenient for you, we can reschedule at a different time. It takes approximately 30 minutes to take the records, which include X-rays of the teeth and the jaw bones; facial and dental photographs; impressions (molds) of upper and lower teeth. A fee will be charged for the diagnostic records.

Consultation

A consultation appointment is usually scheduled about a week after the diagnostic records. Dr. Phoenix will discuss the diagnostic findings, specific treatment objectives, the treatment plan and duration.

Beginning of Treatment

The initial appointment to place the braces or an appliance on your teeth takes approximately 1-2 hours. These longer appointments are scheduled early in the day, so some school time will have to be missed. After starting your treatment we will see you back every 3-6 weeks for adjustments, which usually take about 30 minutes. If you cannot keep an appointment, please notify us immediately, as most appointments fill well in advance.

Financial Arrangements

Treatment fees vary depending on the complexity and duration of the treatment required. At the time of the initial exam we will be able to give you a fairly accurate estimate. Many options are available to make this investment affordable:

Insurance

We accept many insurance plans and we will be pleased to work with you if you have dental insurance that includes orthodontics. At the start of treatment, please provide us with the information about your insurance coverage. Please remember that our financial agreement is with you, and if your company or the carrier changes or cancels the policy or does not abide by the expected terms, the responsibility for the fee for treatment remains yours.
